Milk poisoning kills children in China

Xinhua news agency has reported in the latest food safety scandal to
hit the [Chinese] dairy industry, that 3 children have died & 35
people have become ill from drinking nitrite-tainted milk in China's
north-western Gansu province.
Most of the 35 were children under 14, now being treated at 2
hospitals in Pingliang city, Xinhua said, citing the local government
and health bureau.
"An initial investigation shows all the patients were poisoned by
nitrite after drinking bulk milk provided by 2 local dairy farms,"
Xinhua said.
Nitrite is used for curing meat.
The 2 farms were sealed off and their managers were being
investigated, Xinhua said.
China's food sector has been beset by poisonings and toxin scandals
that have shaken consumer confidence, and dairy production has been at
the heart of those worries.
China's quality inspection agency last week [week of 28 Mar 2011]
ordered nearly half the country's dairy firms to halt production as
part of a campaign to clean up the industry.
In 2008, at least 6 children died and nearly 300 000 fell ill from
powdered milk laced with melamine, an industrial chemical added to
low-quality or diluted milk to fool inspectors by giving misleadingly
high readings for protein levels.
Earlier this year [2011], Chinese quality authorities sought to calm
renewed public alarm after reports that some manufacturers had
illegally added a leather protein powder to dairy products to cheat
protein-content checks.
Zhao Lianhai, who organised a website for parents of children who
became ill from tainted milk after his own son became sick, was jailed
for 2.5 years last November [2010] after being convicted of "inciting
social disorder."

[Sodium nitrite has on a number of occasions been mistaken for table
salt. However, to have it in milk is difficult for me to understand.
While it has been used in the meat industry, its use or purpose in the
milk industry sounds like adulteration of the product.
Clinical signs of ingestion may include gastroenteritis, abdominal
pains, dizziness, bloody diarrhea, convulsions and collapse. Purging
(vomiting) and diuresis are expected. If the victim receives small,
repeated doses there may be headache and mental impairment. Generally
in the case of mistaking sodium nitrite for sodium chloride, the dose
may be larger for large meals typical of celebrations.
Sodium nitrite has been used in curing meat for centuries. Nitrite is
added to certain foods to prevent the growth of the spore-forming
bacterium _Clostridium botulinum_, whose toxin causes botulism,
leading to paralysis and, potentially, death. The word botulinum comes
from the Latin word 'botulus', meaning sausage, which was responsible
for many deaths centuries ago before curing with nitrite was used. In
addition to serving as an antimicrobial, nitrite is used to produce
the characteristic flavor, texture, and pink color of cured meats.
At significant blood levels, the nitrite's oxidation of the iron in
hemoglobin from the ferrous to the ferric state yields methemoglobin,
which does not carry oxygen and imparts a brown hue to the blood.
Symptoms of methemoglobinemia include headaches, fatigue, tachycardia
(fast heart rate), weakness, and dizziness. As oxygenation of tissues
decreases to the point of becoming truly inadequate, dyspnea
(shortness of breath), acidosis, arrhythmias (irregular heart
rhythms), coma, convulsions and even death may occur. - Mod.TG]

A 2nd person has died in the salmonella outbreak linked to DeFusco's
Bakery in Johnston, Rhode Island. The Health Department learned Fri 8
Apr 2011 that a man in his 90s died the previous day in a hospital,
said spokeswoman Annemarie Beardsworth. She declined to identify the
hospital.

The man, who lived in Providence County, reported eating a product
made at DeFusco's, which was purchased on 19 Mar 2011. His symptoms
began on 25 Mar 2011, and he was hospitalized 3 days later. A stool
sample tested positive for _Salmonella_. He was one of the cases the
Health Department had been tracking as part of its outbreak
investigation. He also had underlying health conditions.

The count of people who fell ill with laboratory-confirmed infection
or compatible symptoms between 14 and 28 Mar 2011, considered the time
frame for this outbreak, has now risen to 66. Of those, all but one
reported eating products from DeFusco's, including the other man who
died. The 1st death came on 23 Mar 2011, a man in his 80s who lived in
Providence County. Of the 66 people who got sick, 28 were
hospitalized, an unusually high percentage for an outbreak of
salmonellosis. But no cases have turned up since 28 Mar 2011,
indicating the source of the outbreak is not continuing to make people
ill, Beardsworth said.

The bakery closed down, and its products were recalled, on 25 Mar
2011 after a Health Department inspection found many food-safety
violations. They included storing empty, baked pastry shells in
cardboard boxes that formerly held trays of raw eggs. Tests later
found evidence of the outbreak bacterium in the boxes, probably from
an infected egg that broke.

The Health Department was 1st alerted to the outbreak when 11
nursing-home residents became ill after eating zeppole, a
custard-filled pastry, made by DeFusco's. Neither of the men who died
lived in a nursing home, however. In addition to closing down
DeFusco's, the outbreak has affected business at other bakeries, at
least 2 of which have taken ads assuring their customers that their
products are safe.

Since February 2011, the Reference Laboratory at the Norwegian
Institute of Public Health has identified identical strains of
_Yersinia enterocolitica_ O:9 in 20 patients living in Norway.
Interviews with the patients with yersiniosis led to suspicion of a
particular pre-packaged lettuce mix that was withdrawn from the
market.
Further investigation led to suspicion of several pre-packaged
lettuce mixes purchased in grocery stores. Preliminary investigations
conducted at the Norwegian Veterinary Institute strengthened this
suspicion. The manufacturer has therefore withdrawn a further 9
lettuce mixes from the market. The Norwegian Food Safety Authority
recommends that consumers should not eat these lettuce mixes. The
Norwegian Institute of Public Health is continuing the investigation
in co-operation with the Food Safety Authority and Veterinary
Institute.
Each year, 50-100 cases of yersiniosis are reported to the Norwegian
Institute of Public Health. Most cases occur sporadically and several
cases with a common source rarely occur in Norway.
Yersiniosis is transmitted primarily through food but can transmit
between people in rare cases. The most common symptoms are diarrhea,
fever and abdominal pain, usually lasting for 1-3 weeks. In most
cases, the disease clears up without treatment.

[The 2 species of _Yersinia_ associated with food-borne disease are
_pseudotuberculosis_ and _enterocolitica_. The latter species, as
noted in the posting, can be associated with abdominal pain as a
hallmark symptom. As a mesenteric lymphadenitis, yersiniosis can mimic
appendicitis but may also cause infections of other sites such as
wounds, joints, and the urinary tract.

At least 14 people have died in Madagascar from poisoned fish. Around
120 other people are seriously ill, with some in hospitals, after
ingesting the toxic sardines which had been caught in the
south-western town of Toliara.
Samples of the fish have been sent for analysis.
In previous food poisoning incidents the contaminated sardines had
eaten poisonous seaweed, which now proliferates on local reefs.

SCOTLAND: LEGIONELLOSIS

The [Landmark] hotel's leisure club and spa have been off limits
since [17 Mar 2011] after staff and guests reported feeling unwell
with flu-like symptoms that sparked testing for _Legionella_
infection. As of last night [5 Apr 2011] a total of 112 people who
work in or have visited the Landmark are known to have had a flu-like
illness with respiratory infection. One person remains in Ninewells
Hospital, the only confirmed case of legionnaires' disease in a person
who visited the hotel's leisure club. There are no further confirmed
cases.
While legionnaires' disease is potentially fatal, Pontiac fever is a
milder illness caused by _Legionella_ bacteria. It [Pontiac fever]
causes a flu-like illness, which lasts just a few days. An NHS
[National Health Service] Tayside spokeswoman said last night [5 Apr
2011] it was "very likely" a number of the respiratory infections
reported at Landmark could be caused by Pontiac fever, but it would
take some time to establish if that was the case. "No one has yet been
confirmed as having Pontiac fever because it is difficult to diagnose
and can require up to 3 separate blood tests over a period of several
weeks," she said.
